Irritant contact dermatitis (ICD)

WHAT IT IS

Irritant contact dermatitis (ICD) is an inflammation of the skin and mucous membranes caused by direct contact with chemical, physical or biological agents. Irritants are able to change the barrier function of the skin, resulting in dehydration and inflammation.

WHAT ARE THE CAUSES

The irritants that most frequently cause ICD (irritative contact dermatitis) are water, detergents, surfactants, alkaline or acidic substances, oils, organic solvents, botanical substances, preservatives.
Other factors that may increase susceptibility to ICD are: sex and age, history of eczema and/or atopic dermatitis, anatomical site involved, chemical and physical properties, exposure time and concentration of the irritant, occlusion, climatic factors (cold, humidity…), mechanical stimuli (pressure, friction, abrasion…).

HOW IT MANIFESTS

Typically, manifestations are localised only in the skin area that came into direct contact with the irritant. In most cases, the hands and forearms are affected, but any part of the body can be involved. The inflammatory reaction is usually confined to the site of contact and pain/burning prevails over itching.
The most common symptoms are: erythema (redness), burning, swelling, small blisters, papules and vesicles, xerosis (dryness of the skin), desquamation, hyperkeratosis, rhagades.

• Prevent contact with the allergen (e.g. by wearing protective gloves and suitable clothing to avoid direct contact when handling risky compounds, at work or at home);
